IMPROPER USE OF THE CARBURETOR HEAT BY THE PILOT AND SUBSEQUENT CARBURETOR ICE. CONTRIBUTING FACTORS WERE: CARBURETOR ICING CONDITIONS, SLICK (WET/SNOW COVERED) RUNWAY, AND TREES/BRUSH NEAR THE DEPARTURE END OF THE RUNWAY.
